Medical Assistant and Phlebotomy Program Application

Seats are limited and selection is competitive. Please answer each question thoughtfully. We want to understand your story, why this opportunity matters to you and how it will impact your life. Unfortunately, one-sentence answers don't give us enough insight to consider your application acceptance. Take your time and show effort. Winners will be announced via our social media platform. This acts as a notification of those who did and did not get the scholarship.

There is a $25 application fee required for all applications. Application will not be processed without this fee being paid. This fee allows the submission to be reviewed for consideration in our sponsored certification

program. This fee is not a deposit, tuition payment, or guarantee of selection into the program. Application fees and school registration fees are non-refundable.
